Referral Process

    Referrals for Mid-Lin Day Care can be made by, in the first instance, contacting our office on 01382 506451 or by emailing manager@mid-lin.co.uk  

    Referrals can be made by individuals or family members as well as by carers and health professionals. The referral process is as follows: 

    Referral forms should then be completed by the potential service user or their representative with as much information as possible to help us accommodate their needs and wishes.  Completed forms should be returned to the manager prior to the initial visit. Referral forms can posted, hand delivered or emailed to manager@mid-lin.co.uk 

    Service user (or representative) calls us to arrange a half hour visit.  During this visit, a member of staff will show the potential service user and their carer/representative around the centre.  We will explain what we do and show you around our facilities.

    The potential service user would then be invited for a half day visit, this includes lunch and to try the Day Care facilities and decide whether or not they would like to commence their four week trial period. The fee for a half day visit is £11.00 We do not offer transport for this visit.  Visits are 11:30am to 2:30pm.  

    Once the service user has decided to commence our services, the potential service user would then attend our centre on the agreed day (which would be most suitable for them) one day a week for a four week trial.  The fee for a full day visit is £22.00 per day.  The cost includes pick up on our minibus and drop off after the afternoon session.  Our minibus leaves the centre at 9:30am to collect service users between 10:00am and 11:30am.  The afternoon minibus leaves the centre at 3:30pm to drop off service users at their home.  Service users will arrive home by approximately 5:00pm.  Please note this is an approximate time as this depends on traffic and any diversions in place. 

    A brief review will then take place to see if the service user is happy and wishes to continue attending the centre.  At this point, it may be possible to add an additional day if the service user wishes.
